ASSUMPTION OF THE RISK AND WAIVER OF LIABILITY RELATING TO CORONAVIRUS/COVID-19:
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is contagious and is believed to spread mainly from person-to-person contact.

Queen Fetish will make every effort to keep our business sanitized and to prevent symptomatic employees and dancers from entering the building. However, COVID-19 has been proven to be asymptomatic in some persons. Queen Fetish cannot guarantee that you or your child(ren) will not become infected with COVID-19 or any other unknown virus.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending Queen Fetish parties or events and that such exposure or infection may result in personal injury, illness, permanent disability, and death. By signing this agreement, I agree that my child and any accompanying persons will not enter Queen Fetish parties if we exhibit any symptoms of COVID-19.

I voluntarily agree to assume all the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at Queen Fetish Parties. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Queen Fetish, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs, or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of  Queen Fetish, its employees, agents, and representatives, whether a COVID-19 or other currently unknown virus infection occurs before, during, or after participation in any Queen Fetish program or class.

REGISTRATION FEES: A non-refundable registration fee of $45 for the first dancer and $20 each additional family member is due upon registration. In addition, a deposit of 50% of one installment is due at this time. This is also non-refundable. Payment in full is required for the registration fee, January tuition installment. You will receive a payment confirmation email once your payment has been processed.

WITHDRAWAL POLICY: We require a 30-day written notice via email if you need to withdraw from The Queen Fetish Dance Studio. Please email queenfetishlv@gmail.com to withdraw from our 2024-2025 season. After the 30-day notice, we will terminate your monthly installment.

MAKE-UP CLASSES: If your dancer misses class due to illness or other personal reasons, there is no make up to the missed class in a similar class. There will be no discount or refund given for missed classes. There are no make-ups for classes missed when the studio is officially closed for holidays, etc. All classes missed due to inclement weather can be made up at your convenience using our existing class schedule.

SUMMER REGISTRATION: A full non-refundable payment will be due upon registering for our summer class dates. The appropriate fees will be emailed to you.

YEAR-END SHOW: We produce a wonderful and entertaining end of the year show with all Queen Fetish Dancers. The show week (including dress rehearsal) is tentatively set for May 30 & May 31, 2025. This is an exciting and rewarding part of the season that everyone looks forward to! We encourage all dancers to participate! Stay tuned for Year-End Show details to be released throughout the year in November and April.

YEAR-END SHOW COSTUMES: Costume costs are generally $65.00-$200.00 per costume. Costume fees will be auto-drafted each month on a billling cycle.  After this fee is processed, it is not refundable for any reason. Any costume alterations needed you will be responsible for . If your dancer will not be participating, you are required to notify us by email to queenfetishlc@gmail.com no later than 2 weeks of a show, event or party. Please do not rely on your dancer to inform us that she or he will not be dancing for any events, parties or the in the Year-End Show!

YEAR-END SHOW TICKETS: Tickets to our Year-End Shows will need to be purchased. Dancers performing in the show do not need a ticket. More information on Show Tickets, including when they will be available to purchase, will be included in the Year-End Show packet released mid-April. The ticket cost will be announced in the Spring when more of the show details come out! It is recommended that all dancers sell tickets to attend for free!

DRESS CODE: Proper shoes and attire must be worn to participate in the class. Hair should be pulled back off the face for all classes. A proper bun must be worn. 

GENERAL RULES:  Please discuss these rules with your dancers! Thank you!

• No food or drinks other than water inside the studios. No exceptions!
• No cell phones, smart watches (like Apple Watch), smart earphones (like Air Pods) are allowed in the dance rooms at any time. If they are inside a dance bag, they should be turned off. If dancers are wearing these in class, they will be asked to remove and take to front desk staff for safekeeping until class is over.
• Our studio is not a large place, so please come serious and ready to rehearse!
• Please make an effort to be prompt! Excessive tardiness will result in additional fees.
• No parents, friends, or siblings are allowed in the classroom at any time. Total focus on you is important. They will learn more if we have their complete attention.
• If a dancer drops a class, a 30-day written notice is required. You will be responsible for payment until the written notice is received and the 30-day period has passed.
• For the safety of our dancers, we typically follow local school and city decisions on inclement weather closings. However, you should always use your own judgement when deciding to travel due to weather. We will confirm our closing via email when inclement weather affects Queen Fetish Studio. We will also communicate all schedule changes on social media as well! No refunds for inclement weather.
